Code Chapter 2600 for personal care homes, 55 Pa. Code Chapter 2800 for assisted living residences) with the home’s plan of correction. A complaint inspection with no violations means DHS cited nothing at that visit. Violation titles are DHS’s own short names for each rule. A provisional license is issued when a home is in substantial but not complete compliance with the rules; a home can hold up to four in a row, six months each. When DHS gives notice of a fine, the fine is charged per resident per day only if the violation is not corrected by the deadline. Violation classes: Class I means a substantial probability of death or serious harm, Class II a substantial adverse effect on health, safety or well-being, Class III a minimal effect.
